29 CFR 1926.300(b)(2): Moving parts, i.e. belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded: a) 132 Beverly Hills Terrace, Woodbridge, NJ 07095. Employees were working on or in close proximity to a gas operated compressor with the guard missing over the pulleys and belt drives exposing them to amputation hazards. Condition occurred on or about 05/13/14.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) 132 Beverly Hills Terrace, Woodbridge, NJ 07095. Employees were allowed to work on and from the roof of a two story apartment complex doing rip and tear work of roofing materials with no fall protection systems in place exposing them to fall hazards. Condition occurred on or about 05/13/14.

29 CFR 1926.1053(b)(1): Portable ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. The portable ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) 132 Beverly Hills Terrace, Woodbridge, NJ 07095. Employees were using a 32 foot Lousisville aluminum extension ladder to access the roof when the ladder did not extend three feet above the landing. Condition occurred on or about 05/13/14.
