Observed on or about 05/13/2014.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Jobsite: 17th and Carpenter Streets - Employees engaged in bricklaying operations were using a portable extension ladder to access the working platforms of a fabricated frame scaffold. The ladder side rails did not extend at least (3) feet above the upper working surface, exposing the employees to fall hazards of approximately thirty (30) feet. Observed on or about 05/13/2014.
