29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross-braces were used as a means of access.   On or about May 14, 2014, employees were not provided a ladder to access the second tier of a scaffold and used the end frames and cross bracing from the lower platform exposing employees to a fall hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   On or about May 14, 2014, employees engaged in masonry activities on a residential home while working from a tubular welded frame scaffold were exposed to falling more than 12 feet to the lower level.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed.   On or about May 14, 2014, employees using a A-frame ladder in a folded position to access the first tier of a two tiered scaffold were exposed to a fall hazard.
