No chemical name was visible and the hazard warning numbers were barely legible.    (b)  In the Handsewn Area, a safety container filled with MEK was not properly labeled.  No chemical name or hazard warning numbers was visible.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a) In the Handsewn Area, a Herbert Jaffe, Inc. tassel machine (Needle:  134_33D) had an opening greater than 1/4" between the needle and shoe exposing employees to struck-by hazards.      The Allen Edmonds Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 111363, citation number1, item number 2 and was affirmed as a final order on November 13, 2012, with respect to a workplace located at 3820 Highway KW, Port Washington, WI.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  (a)  The employer had not accomplished a periodic inspection of the lockout / tagout procedures.  Accomplishing a review of lockout tagout procedures ensures continuity of procedures and proper application.  Inadequate review of changes to equipment and machinery can lead to improper lockout.
