29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a)  A Hamilton Beach microwave was plugged into an extension cord for longer than 90 days, violating the extension cords UL listing and labeling for temporary power.   (c)  3rd Floor Housekeeping Room; A Keurig coffee maker was plugged into a relocatable power tap. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.    (d)  Slot Technician trailer;  A Magic Chef microwave was plugged into a relocatable power tap. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  a)  13 Moons kitchen:  The two small rear rooms did not have a view of an exit sign or exit door, thus a directional sign was needed.   b)  13 Moons kitchen;  An exit sign was not located over the double doors.  c)  Workshop;  Three doors were located in the workshop and the door which lead to the nearest exit corridor did not have an exit sign.

29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.  a)  Casino Loading Area;  A giant grill and other restaurant equipment were stored above an office space, and the load rating for the overhead storage area was not posted.   b)  Casino Loading Area;  Several boxes of filters for the HVAC system were stored above the Cold Storage Refrigeration structure, and the load rating for the overhead storage was not posted.   c)  Casino Loading Area;  Lockers and other restaurant supplies were stored above a work area, and the load rating for the overhead storage area was not posted.
