Worksite located at 335 N. Main, Nevada, Missouri- Employees were observed working and walking on a steep roof and were not protected from falling by the use of any means of fall protection.

Main, Nevada, Missouri- Employees were exposed to a fall hazard in that a step-ladder was being used on a pitched roof in order to access the upper roof level. The step-ladder was subject to falling in that the feet on the ladder are not designed to be placed on a pitched roof.

Worksite located at 335 N. Main, Nevada, Missouri- Employees were exposed to fall hazards in that extension ladders and step ladder being used on the site did not extend at least 3 feet above the upper landing surfaces.
