29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) Northwest Side of House - On or about 05/14/2014, a mason tender was exposed to a 14 foot fall from a wooden pallet which was elevated by the forks of a all-terrain fork lift for the pallet was not secured to the lift's forks and the worker was not provided with fall protection.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Northwest Corner of House - On or about 05/14/2014, a mason tender was not provided with fall protection and exposed to a 14 foot fall from an unguarded platform which was elevated by an all-terrain powered industrial truck. b) Northwest Corner of House - On or about 05/14/2014, masons were exposed to a 14 foot fall from the platform of a fabricated frame scaffold which had not been provided with end rails on either end of the platform. c) Northwest Corner of House - On or about 05/14/2014, masons were exposed to a 14 foot fall from the platform of a fabricated frame scaffold which had not been provided with guardrails in an area facing the building and where no overhand bricklaying was taking place. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
