29 CFR 1926.451(b)(2)(i): Each ladder jack scaffold, top plate bracket scaffold, roof bracket scaffold, and pump jack scaffold was not at least 12 inches (30 cm) wide: (a) Front side, new residential construction located at 25 Bryce Creek Drive, Newnan, GA 30265: The ladder jack scaffold platform provided was not at least 12-inches wide as a 2x10 which measures approximately 9-1/2-inches wide was used which exposed the siding mechanic to falls of approximately 12-feet, 9-inches to the lower level.

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another. a) Front side, new residential construction located at 25 Bryce Creek Drive, Newnan, GA 30265: The employer bridged one scaffold plank onto another scaffold plank to make a longer work platform without using an additional ladder jack support. Employees using this scaffold system were exposed to falls of approximately 12-feet, 9-inches to the lower level as scaffold platforms cannot be bridged on to another.

(a) Front side, new residential construction located at 25 Bryce Creek Drive, Newnan, GA 30265: Employer was exposed to falls of approximately 12-feet, 9-inches while installing vinyl soffit from a ladder jack scaffold platform with no fall protection system in place. Benjamin Olivares was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 711258, citation number 1, item number 5 and was affirmed as a final order on 05/02/2013, with respect to a workplace located at 101 Broken Bit Circle (Lot #279), Guyton, GA 31312.
