29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about May 14, 2014 and at times prior to, employees, who were engaged in roof work on a commercial building were not protected from falling more than 6-feet to the concrete below.

29 CFR 1926.1053(b)(1): The employer did not ensure that when portable ladders were used for access an upper landing surface, the ladder side rails extended at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about May 14, 2014 and at times prior to, employees, who were engaged in roof work on a commercial building were exposed to a fall of approximately 12 feet to the concrete below when using a ladder that did not extend at least 3 feet above the landing.
