29 CFR 1910.37(b)(6):  Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source:  The Garage, south wall.  On or about May 15, 2014, employees were exposed to, in the event of an emergency, delayed egress from the building, as the fire exit door sign was not illuminated, while they worked maintaining vehicles.

29 CFR 1910.212(a)(3)(ii):   The point of operation of machines whose operation exposes an employee to injury, was not guarded:  East wall of garage.  Central Machinery, 16-Speed Heavy Duty drill Press, Model No. T-6725, Serial No.  779716.  On or about May 15, 2014, employees were exposed to an amputation hazard from the lack of a guard at the point of operation, while drilling 5/8-inch holes in metal brackets, some measuring 5-inch x 10-inches.

29 CFR 1910.215(b)(9):   The distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceeded one-fourth inch:  South wall, garage.  Challenger Bench Grinder, by Wilton, Model No. 160, Serial No. 407342.  On or about May 15, 2014, and at times prior thereto, employees were exposed to a struck-by hazard, as the tongue is approximately 1 inch from the abrasive wheel, while deburring metal parts.
