Panetta Roofing & Siding, Inc., 1540 West 26th Street, Erie, PA  16508, west side of the building, on or about May 14, 2014 and times prior thereto:  Employees were utilizing a warning line system as a means for fall protection while conducting roof activities at a height of 19.3 feet above ground level.  An access path formed by two warning lines was not erected from the warning line area to the ladder access on the west side of the building.
