29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  a.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, June 18, 2015, for a violation of 29 CFR 1926.501(b)(13), issued as citation 2, item 1, and described as follows:       On the roof where employees were installing shingles, the employees were wearing safety harnesses but were not tied off, thereby exposing employees to a fall hazard.       The Julio Galindo Company  was previously cited for a violation of this occupational safety and health standard, 29CFR 1926.501(b)(13), contained in OSHA inspection number 604918, citation number 1, item number 2, and affirmed as a final order on November 13, 2012, with respect to a workplace located at 8297 Priestly Avenue in Reynoldsburg, Ohio.     The Julio Galindo Company  was previously cited for a violation of this occupational safety and health standard, 29CFR 1926.501(b)(13), contained in OSHA inspection number 900356, citation number 1, item number 2, and affirmed as a final order on June 10, 2013, with respect to a workplace located at 6643 Brixton Park Avenue in Columbus, Ohio.

29 CFR 1926.502(d)(11): Lifelines shall be protected against being cut or abraded.  a.   On the roof where employees tied back the lifelines around the 2 x 4 board at the peak of the roof, the ropes were cut and abraded and damaged thereby exposing employees to a fall hazard.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  29 CFR 1926.502(d)(15)(i): as part of a complete personal fall arrest system which maintains a safety factor of at least two.  a.     On the Zone 2 roof where shingles were applied, the employer did not provide a anchor for employees who were on the roof attached to a lifeline without an anchor thereby exposing employees to a fall hazard.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system.    a.
