This violation was observed on or about May 15, 2014, in Building G-1 where employees were exposed to being struck-by loads lifted with a 500-lbs pedestal jib crane at or near CNC Lathe 25 without an annual inspection being conducted and records maintained.      c. This violation was observed on or about May 15, 2014, in Building G-1 where employees were exposed to being struck-by loads lifted with a 500-lbs. pedestal jib crane at or near CNC Lathe 27 without an annual inspection being conducted and records maintained.     d. This violation was observed on or about May 15, 2014, in Building G-1 where employees were exposed to being struck-by loads lifted with a 500-lbs. wall-supported jib crane at or near CNC Lathe 29B without an annual inspection being conducted and records maintained.     e. This violation was observed on or about May 15, 2014, in Building G-1 where employees were exposed to being struck-by loads lifted with a 500-lbs. wall-supported jib crane at or near CNC Lathe 11A without an annual inspection being conducted and records maintained.     f. This violation was observed on or about May 15, 2014, in Building G-1 where employees were exposed to being struck-by loads lifted with a 500-lbs. pedestal jib crane at or near CNC Mills 25, 6, & 5A without an annual inspection being conducted and records maintained.     g. This violation was observed on or about May 15, 2014, in Building G-1 where employees were exposed to being struck-by loads lifted with a 500-lbs. wall-supported jib crane at or near CNC Lathes 29A & 30 without an annual inspection being conducted and records maintained.     h. This violation was observed on or about May 15, 2014, in Building G-1 where employees were exposed to being struck-by loads lifted with a 500-lbs. pedestal jib crane at or near CNC Mills 11B & 11C without an annual inspection being conducted and records maintained.     i. This violation was observed on or about May 15, 2014, in Building G-2 where employees were exposed to being struck-by loads lifted with a 500-lbs. wall-supported jib crane at or near CNC Lathe 4B without an annual inspection being conducted and records maintained.      j. This violation was observed on or about May 15, 2014, in Building G-2 where employees were exposed to being struck-by loads lifted with a 500-lb. beam-supported overhead crane at or near CNC Lathe 5B without an annual inspection being conducted and records maintained.     k. This violation was observed on or about May 15, 2014, in Building G-2 where employees were exposed to being struck-by loads lifted with a 500-lb. beam-supported overhead crane at or near CNC Lathe 4C without an annual inspection being conducted and records maintained.     l. This violation was observed on or about May 15, 2014, in Building G-2 where employees were exposed to being struck-by loads lifted with a 500-lb. beam-supported overhead crane inside the QC room without an annual inspection being conducted and records maintained.     m. This violation was observed on or about May 15, 2014, in Building G-2 where employees were exposed to being struck-by loads lifted with a 500-lb. beam-supported jib crane at or near CNC Mill 10A without an annual inspection being conducted and records maintained.     n. This violation was observed on or about May 15, 2014, in Building G-2 where employees were exposed to being struck-by loads lifted with a 500-lb. pedestal jib crane at or near a Doosan CNC Mill, S/N MV0010-002300, without an annual inspection being conducted and records maintained.      o. This violation was observed on or about May 15, 2014, in Building G-2 where employees were exposed to being struck-by loads lifted with a 1500-lb. pedestal jib crane at or near CNC Mill 2B without an annual inspection being conducted and records maintained.      p. This violation was observed on or about May 15, 2014, in Building G-2 where employees were exposed to being struck-by loads lifted with a 500-lb. pedestal jib crane at or near CNC Mill 2A without an annual inspection being conducted and records maintained.      q. This violation was observed on or about May 15, 2014, in Building G-2 where employees were exposed to being struck-by loads lifted with a 500-lb. pedestal jib crane at or near the de-burr table without an annual inspection being conducted and records maintained.      r. This violation was observed on or about May 15, 2014, in Building G-2 where employees were exposed to being struck-by loads lifted with a 500-lb. pedestal jib crane at or near CNC Mill 10B without an annual inspection being conducted and records maintained.      s. This violation was observed on or about May 15, 2014, between Building G-2 and G-3 where employees in the Band saw area were exposed to being struck-by loads lifted with a 1000-lb. pedestal jib crane without an annual inspection being conducted and records maintained.      t. This violation was observed on or about May 15, 2014, in Building G-3 where employees were exposed to being struck-by loads lifted with a 500-lb. pedestal jib crane without an annual inspection being conducted and records maintained.      u. This violation was observed on or about May 15, 2014, in Building G-4 where employees were exposed to being struck-by loads lifted with a 500-lb. pedestal jib crane at or between CNC Mill 3A & 3B without an annual inspection being conducted and records maintained.     v. This violation was observed on or about May 15, 2014, in Building G-4 where employees were exposed to being struck-by loads lifted with a 500-lb. pedestal jib crane at or near CNC Mill 3C without an annual inspection being conducted and records maintained.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation describing the actions it is taking to ensure that employees are not exposed to struck-by hazards.

This violation occurred on or about May 15, 2014, and times prior thereto, in the maintenance area between Building G-2 and G-3 where employees were exposed to electrical and struck-by hazards when performing servicing/maintenance on a HEM H90A-B/F Band Saw, S/N 597898 without procedures developed, documented and utilized to control potentially hazardous energy.     z. This violation occurred on or about May 15, 2014, and times prior thereto, in the maintenance area between Building G-2 and G-3 where employees were exposed to electrical and struck-by hazards when performing servicing/maintenance on a HEM H90A-1 Band Saw, S/N 915305 without procedures developed, documented and utilized to control potentially hazardous energy.    aa. This violation occurred on or about May 15, 2014, and times prior thereto, in Building G-3 where employees were exposed to electrical and struck-by hazards when engaged in servicing/maintenance activities including setting up a Vanguard Manual Lathe, Model CA6266C, S/N A40719295, without procedures developed, documented and utilized to control potentially hazardous energy.         bb. This violation occurred on or about May 15, 2014, and times prior thereto, in Building G-3 where employees were exposed to electrical and struck-by hazards when engaged in servicing/maintenance activities including setting up a Mori Seiki Manual Lathe without procedures developed, documented and utilized to control potentially hazardous energy.     cc. This violation occurred on or about May 15, 2014, and times prior thereto, in Building G-3 where employees were exposed to electrical and struck-by hazards when engaged in servicing/maintenance activities including setting up a Kingston Manual Lathe, Model CH93928, without procedures developed, documented and utilized to control potentially hazardous energy.     dd. This violation occurred on or about May 15, 2014, and times prior thereto, in Building G-3 where employees were exposed to struck-by hazards when operating a Wells Index Mill, Model 047, SN 19332, without a point of operation guard.    c. This violation occurred on or about May 15, 2014, and times prior thereto, in Building G-3 where employees were exposed to struck-by hazards when operating a Wells Index Mill, Model 047, SN 22064, without a point of operation guard.
