29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  Jobsite: Site.  Employees were exposed to being struck by falling objects such as nail guns, hand tools, tape measures, and plywood while working directly below construction activities where head protection was not utilized.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  Jobsite: Site.  a) Employees were exposed to falls of up to 25 feet to the ground below while installing sheathing on the 8-pitch roof where fall protection had not been provided.  b) Employees were exposed to falls of up to 20 feet to the ground below while working/walking on the 2nd and 3rd floors where the several rough window openings, with 25 inch high sills, had not been not guarded.  c) Employees were exposed to a fall of approximately 9 feet into the basement where a floor hole for the unfinished stairway to the basement, which measured 60 inches by 45 inches, was unguarded.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  Jobsite: Front and rear entrances/exits.  Employees were exposed to slips, trips, and falls while accessing/egressing the building where narrow chicken ladders had been fashioned to go from the ground to the floor which was an approximately 3 feet break in elevation.

Employees were exposed to falls of up to 8 feet to the floor below while accessing/egressing their work areas where the stairs to the 2nd and 3rd floors, comprised of 14 risers, were not equipped with a hand rail.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926:(a):  Jobsite: 3rd floor.  Employees were exposed to falling approximately 10 feet onto the stairs and floor below while working/walking near an unguarded stairway landing, measuring 128 inches by 39 inches, on the 3rd floor.
