29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts. a) Fiber Building Department - On or about 05/15/14 and at times prior thereto employees working at the end of line D were exposed to ingoing nip points created by the 7.0 in. diameter rotating cylinder and the stationary steel bar attached to the frame of the machine. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Fiber Building Department - On or about 05/15/14 and at times prior thereto the guard for the guillotine at Line F did not protect the workers from contact with the blade. b) Fiber Building Department - On or about 05/15/14 and at times prior thereto the guard for the guillotine at Line E did not protect the workers from contact with the blade. c) Fiber Building Department - On or about 05/15/14 and at times prior thereto the guard for the guillotine at Line D did not protect the workers from contact with the blade. d) Fiber Building Department - On or about 05/15/14 and at times prior thereto the guard for the guillotine at Line C did not protect the workers from contact with the blade. e) Fiber Building Department - On or about 05/15/14 and at times prior thereto the guard for the guillotine at Line 3 did not protect the workers from contact with the blade.
