Jermunfran, Inc., Colonna's Shipyard, Warping Tug:  On or about 05/15/14, an employee spraying the Hempel Hempadur paint was inappropriately wearing a half face air purifying respirator, in that the manufacturer recommended an air supplied respirator.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a.)  Jermunfran, Inc., Colonna's Shipyard, Ferry Pocahontas, Void 3A:  On or about 05/12/14, the Dewalt handheld grinder was not used in accordance with instructions included in the listing and labeling, in that the employer removed the power cord and hardwired an extension cord to supply power, creating a contact with electrical current hazard.  b.)  Jermunfran, Inc., Colonna's Shipyard, Ferry Pocahontas, Void 3A:  On or about 05/12/14, the extension cord supplying power to the Dewalt grinder was not used in accordance with instructions included in the listing and labeling, in that the employer removed the female end of the cord and wired it into the grinder, creating a contact with electrical current hazard.  c.)  Jermunfran, Inc., Colonna's Shipyard, Ferry Pocahontas, Void 3A:  On or about 05/12/14, extension cords were not used in accordance with instructions included in the listing and labeling, in that they were daisy chained to supply power to handheld grinders and portable lights, creating a fire hazard.
