29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  On or about May 16, 2014, and at times prior thereto, the employer did not ensure that the first and second tiers of the tubular welded frame scaffold system were fully planked. The first tier was missing approximately 3 feet of planks between the scaffolding uprights and the second tiers was missing approximately 3 feet of planks between the scaffolding uprights in two locations.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  On or about May 16, 2014, and at times prior thereto, the employer did not ensure that each scaffolding planks were installed so that there was not a space greater than one inch between the adjacent planks. On the first level there were three instances where the gaps were approximately 4 inches up to 8 inches wide. On the second level there were four instances where the gaps were approximately 4 inches wide.
