29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) On or about May 6th, 2014, at a job site located at 12900 E 350 Highway in Kansas City, MO, employees were exposed to potential electrical shocks and burns in that a 3-wire extension cord used to power a Flotec water extraction pump had no grounding pin. Abatement certification for this Item is not required.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth: (a) On or about May 6th, 2014, employees were exposed to possible strains and sprains while getting into and out of a trench excavation more than 4 feet in depth in that a suitable means of access and egress was not provided. One employee was observed climbing up and over the east vertical trench wall and the spoil pile to exit the trench. A short A-frame ladder against the west trench wall was not adequate in that it was being used in its folded position (contrary to its designed usage) and did not extend at least 3 feet above the upper landing surface. Abatement certification for this Item is not required.
