Cherry Street, Glendale, CO 80246:  On and before May 15th, 2014, the controlling contractor in charge of the worksite did not ensure the open sided floor next to the job made wood ladder between level 2 and 3 was equipped with guardrails. The condition exposed employee(s) of a sub-contractor to a fall hazard up to approximately 10 feet.     (b)Reylenn Construction, Inc. at 801 S. Cherry Street, Glendale, CO 80246:  On and before May 15th, 2014, the controlling contractor in charge of the worksite did not ensure the open sided floor located on the west side of level 3 was equipped with suitable fall protection. The condition exposed a subcontractors employee installing 2X8 sill plates over a trash room block wall to a fall hazard of approximately 30 feet.
