29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: (a)Clay Dean Electric, LLC at 801 Cherry Street, Glendale, CO 80246: On and before May 15th, 2014, the two 240 volt outdoor electrical cables used to provide temporary electrical power to the general contractor jobsite trailer(s) were not protected against damage. The two electrical cables were placed along the gate opening used by vehicles entering and exiting the worksite. As a result, the two electrical cables were substantially flattened from vehicles driving over the cables.

29 CFR 1926.405(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed so as to prevent moisture or water from entering and accumulating within the enclosures: (a)Clay Dean Electric, LLC at 801 Cherry Street, Glendale, CO 80246: On and before May 15th, 2014, the two 240 volt outdoor electrical junction boxes used to provide temporary electrical power to the general contractor trailer(s) were not equipped with appropriate strain relief fittings. The strain relief fittings were not rated for damp and/or wet applications. Condition allowed moisture and or water to enter electrical boxes. (b)Clay Dean Electric, LLC at 801 Cherry Street, Glendale, CO 80246: On and before May 15th, 2014, the 240 volt electrical junction box used to provide temporary electrical power to the general contractor trailer(s) was not rated for damp or wet locations. Condition allowed moisture and or water to enter the metal electrical box.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. (a)Clay Dean Electric, LLC at 801 Cherry Street, Glendale, CO 80246: On and before May 15th, 2014, the two 240 volt outdoor electrical cable boxes used to provide temporary electrical power to the general contractor jobsite trailer(s) were missing appropriate strain relief fittings. The outer sheath on both cables did not pass into and through the strain relief fittings.
