On or about 16 May, 2014, at the above addressed job site, an employee was exposed to the hazard of falling while working from a ladder that was staged on a piece of wood plank which was stretched between the second floor patio and the roof of the garage.

On or about 16 May, 2014, at the above addressed job site, an employee was exposed to the hazard of falling while working from a "A" frame ladder that staged on a elevated wood plank and was leaning against the face a home.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   a. On or about 16 May, 2014, at the above addressed job site, employees were exposed to the hazard of falling while working from a wood plank that was stretched between two elevated points of a home under construction. The wood plank stretched from the second floor patio to the roof of the garage.

On or about 16 May, 2014, at the above addressed job site, employees were exposed to the hazard of falling while using a staircase which had an handrail support system installed which would not withstand a 200lb force, if it were applied.
