29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): On or about May 15, 2014, at the worksite located at 100 South University in Little Rock, Arkansas, on the supported scaffold erected on the south side of the building beneath the flat roof canopy, the platform that employees worked from to perform EFIS application tasks was not decked the full width of the scaffold. Employees were exposed to the hazard of a scaffold collapse and/or falls from elevation.

29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement: On or about May 15, 2014, at the worksite located at 100 South University in Little Rock, Arkansas, on the supported scaffold erected on the south side of the building, (A) the footing on the southwest end of the scaffold was positioned adjacent to a trench. (B) footings were not centered on the wooden boards that were used to support the scaffold system. These conditions exposed employees to the hazard of a scaffold collapse and/or falls from elevation.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about May 15, 2014, at the worksite located at 100 South University in Little Rock, Arkansas, on the supported scaffold erected on the west side of the building, guardrails were not provided on all open sides and ends to protect employees engaged in activities such as but not limited to EFIS application tasks. Employees were exposed to the hazard of a scaffold collapse and/or unprotected falls greater than 6 feet.
