29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:     (a) On or about May 14, 2014, Bighorn Rain Gutter, Inc., did not ensure that portable ladders, used to access upper landing surfaces, extend at least 3 feet above the upper landing surface to which the ladders were used to gain access. Workers were observed using a ladder to access the roof of a residential building. Ladder did not extend at least 3 ft above the landing platform and was not tied to the roof edge. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:       (a) On or about May 14, 2014, Bighorn Rain Gutter, Inc., did not ensure that its employees were protected from falls while working at heights greater than 6 feet. Employees of Bighorn Rain Gutter, Inc. were exposed to fall hazards in excess of 11 feet while engaged in roofing activities on the roof of a residential building with no fall protection in place.         Bighorn Rain Gutter, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 51702, citation number 1, item number 1 and was affirmed as a final order on August 7, 2012 with respect to a workplace located at 3813 Cedarwood Lane, Johnstown, CO 80534.       Bighorn Rain Gutter, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 312133846, citation number 1, item number 1(b) and was affirmed as a final order on August 19, 2008, with respect to a workplace located at 6440 Xanadu Way, Centennial, CO 80111.
