29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems:  On or about May 16, 2014, at 101 S. Camelia, Fort Valley, Georgia:  Employees performing storm drain installation work were exposed to cave-in hazards when they worked inside an approximately 35-foot long, 12-foot wide, 10.3-foot deep excavation that had not been inspected prior to employees entering the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that th(a)(LOCATION)(DIMENSIONS OF THE EXCAVATION)(EMPLOYEES EXPOSED)(THE EMPLOYER HAD NOT MET ALL THE ELEMENTS OF SLOPING/BENCHING OPTION (2), -3 OR (4)):  On or about May 16, 2014, at 101 S. Camelia, Fort Valley, Georgia:  Employees performing storm drain installation were exposed to cave-in hazards while working in an approximately 35-foot long, 12-foot wide, 10.3-foot deep excavation that did not have a protective system installed.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 16, 2014, at 101 S. Camelia, Fort Valley, Georgia:  Employees performing storm drain installation work were exposed to a fall hazard of approximately 10.3-feet when they used a Werner 20-foot aluminum extension ladder for access and egress to and from the excavation they worked in that was not extended three feet above the excavation opening.
