29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:  (a) On or about May 16, 2014 employees were cutting metal using lances and were exposed to fumes, but a hazard assessment had not been done to determine if decontamination procedures and personal protective equipment was adequate.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.1025(i)(2)(i): Clean change rooms were not provided for employees exposed to lead in excess of the permissible exposure limit (PEL), without regard to the use of respirators:  (a)  At Springfield Iron & Metal Canyon Commercial Construction failed to provide clean change rooms for employees to change at the beginning and end of the work shift.  (1) On or about May 16, 2014 an employees of Canyon Commercial Construction were cutting non-ferrous scrap metal which exposed employees to lead.  The employees did not have a clean change room on site and had to leave the site and travel to another location off site to change clothes at the beginning and end of the work day.  Previous sampling by OSHA showed that workers doing this job were exposed to airborne concentrations of lead at 1.72 times the OSHA permissible exposure level (PEL) of 50 ug/m3.

29 CFR 1910.1025(i)(3)(i): Employee(s) exposed to lead in excess of the permissible exposure limit (PEL), without regard to the use of respirators, were not required to shower at the end of the work shift:  (a)  At Springfield Iron & Metal Canyon Commercial Construction failed to provide a shower facility for the employees to shower at the end of the work shift.  Workers were forced to leave the site and travel to another location off site to shower.  On or about May 16, 2014 an employee of Canyon Commercial Construction was observed cutting non-ferrous scrap metal.   Previous sampling by OSHA showed that workers doing this job were exposed to airborne concentrations of lead at 1.72 times the OSHA permissible exposure level (PEL) of 50 ug/m3.
