29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition: a) North side of the building: An employee sanding EPS insulation board which was attached to the wall of the building was not wearing safety glasses. The EPS insulation MSDS required the use of safety glasses while sanding on it. b) North side of the building: An employee sanding EPS insulation board which was attached to the wall of the building was wearing his respirator down over his chin. The EPS insulation MSDS required the use of the respirator while sanding on it.

29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units: a) Building 626: The tubular welded frame scaffold erected on all four sides of the building had at least 12 instances of scaffold legs on the west and south sides sitting atop unstable basing in the form of, stacks of scrap wooden boards placed atop voids. ABATEMENT CERTIFICATION IS REQUIRED
