29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  At this location, employees performed brick mason work from the top of a scaffold at a height greater than forty feet and the scaffold poles did not bear on base plates or mudsills exposing employees to a fall hazard to the concert floor.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  At this location, employees conducting masonry work from the top of a fixed scaffold at a height greater than forty feet above concrete ground level were not provided with adequate fall protection such as but not limited to a guardrail system.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks:  At this site, an employee operated an a powered industrial truck to move pallets of brick.  The truck seat belt was not operational.
