29 CFR 1926.453(b)(2)(v): A personal fall arrest system was not worn and a lanyard attached to the boom or basket when working from an aerial lift. South side of the building; employees installed sheathing from a JLG Lift, model 400S, without the use a personal fall arrest system, exposing the employee to fall hazards of approximately 12 feet.

29 CFR 1910.1200(e)(1): Employers did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met. South side of the building; the employer did not develop and implement a hazard communication program for glass sheathing that contained crystalline silica, exposing employees to inhalation hazard.
