OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees where employees were exposed to burns and struck-by hazards while operating a casting machine: (a) On or about May 19, 2014, in the foundry department, the bolts used on Spinner B centrifugal casting machine to hold the die to the rotating turntable were not torqued to manufacturer's specifications, bolt loads were not calculated and the bolts were reused without certain limits verified. The die containing molten metal on Spinner B centrifugal casting machine became disengaged from the turntable, causing catastrophic failure.
