29 CFR 1926.21(b)(2):  The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to their work environment to control or eliminate any hazards or other exposure to illness or injury:     Site - Employees accessing the roof by means of an aluminum extension ladder were not adequately trained to be protected from fall hazards when a break in elevation of 27 inches between the top of a parapet wall and the roof level was not equipped with a ladder or stairway. Employees needed to climb onto the parapet wall to gain access to and from the ladder which was the only access point to the roof.
