29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinder is designed to accept a cap, were not in place, hand-tight, except when cylinders are in use or connected for use: Near the front of the warehouse, north side, on or about May 20, 2014, and at times prior thereto, employees were exposed to burns and injuries from fire and explosion, as the cylinders of oxygen and acetylene were not capped when not in use.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: Near the front of the warehouse, north side, on or about May 20, 2014, and at times prior thereto, employees were exposed to burns and injuries from fire and explosion, as the cylinders of oxygen and acetylene were stored directly adjacent to each other, as they worked performing metal fabrication and distributing activities.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded to prevent the operator from having any part of his body in the danger zone during the operating cycle: Atlantic Machine Tools shear, Model HDE10-14, Serial 97075070. Near the center of the warehouse, south wall, on or about May 20, 2014, and at times prior thereto, employees were exposed to a caught-in hazard at the unguarded rear area of the 11 1/2-foot shear, while they retrieved supplies from the rack for customers.
