29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about May 19, 2014, and at times prior thereto, employees were using a DeWalt Sawzall to cut electrical conduits where the portable extension cord connected to a Powerhorse 2200 generator had a broken/missing ground pin exposing employees to electrical shock hazards.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: On or about May 19, 2014, and at times prior thereto, employees performing utility work inside of a trench were not provided with a ladder for egress exposing the employees to entrapment and/or engulfment hazards.
