29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter:  a)  Bulk Chemicals, Inc. - The employer did not ensure that an employee performing the Powder Mixing operation was fit tested on an annual basis, on or about May 27, 2014. 29 CFR 1910.1026(d)(2)(v): When periodic monitoring indicated that employee exposures were below the action level, and the result was confirmed by the result of another monitoring taken at least seven days later, the employer may then discontinue the monitoring for those employees whose exposures were represented by such monitoring:  a)  Bulk Chemicals, Inc. - A Powder Mixing employee's exposure level to hexavalent chromium exceeded the permissible exposure limit of 5 ug/M3 on both April 17, 2013 and August 8, 2013 and was below the action level on November 22, 2013, but the employer did not conduct additional air monitoring to confirm the test result for the powder mixing operation, on or about May 27, 2014.  No abatement certification or documentation required.

29 CFR 1910.1026(m)(1)(ii): The employer did not maintain an accurate record of all air monitoring which included at least the information contained in paragraphs 29 CFR 1910.1026(m)(1)(ii)(A) through 29 CFR 1910.1026(m)(1)(ii)((F):  a)  Bulk Chemicals, Inc. - The employer's air monitoring data for hexavalent chromium did not include the operation involving the exposure to chromium (VI) that was being monitored, the type of personal protective equipment worn, and the name, social security number, and job classification of all employee represented by the monitoring, on or about May 15, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
