29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Neyra Job Site, South Windsor - On or about 5-20-14 - Employees were removing old gutters and installing wood flashing and new gutters which exposed the employee to falls of 17 feet to 24 feet with no fall protection system in use.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Neyra Job Site, South Windsor - On or about 5-20-14 - Employees were using a 24 foot extension ladder to access the 17 foot high roof of the building which extended only 18 to 20 inches above the roof.
