29 CFR 1926.501(b)(1): The employer did not ensure that each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a) Lakeside Exteriors, Inc. @ 1060 Greystone Manor Parkway, Chesterfield, MO 63005: On May 21, 2014, the employer did not ensure that employees were protected against fall hazards. Two employees installed siding on a two-story, residential home from a ladder jack scaffold without the use of a guardrails or personal fall arrest systems. This condition exposed employees to a fall hazard of approximately 14 feet 4 inches.   Lakeside Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314623612, citation number 1, item number 1 and was affirmed as a final order on December 6, 2010, with respect to a workplace located at 250 South Brentwood Blvd., Clayton, MO 63105.
