29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   a) Jobsite, 5812 Library Road, Bethel Park, PA:  On or about May 22, 2014, 3 employees were putting paper down on a commercial roof without fall protection and were exposed to a fall hazard of approximately 22 feet to the ground.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) Jobsite, 5812 Library Road, Bethel Park, PA:  On or about May 22, 2014, 3 employees were putting paper down on a commercial roof and were not protected from falling.  The foreman said the employees were trained in fall protection, but did not have any written documentation.  The fall hazard to the employees was approximately 22 feet to the ground.
