The written procedure for the lockout/tagout of this switchgear unit was not prepared until after the arc flash occurred.

29 CFR 1910.333(b)(2)(ii)(B): The circuits and equipment to be worked on were not disconnected from all electric energy sources. Control circuit devices, such as push buttons, selector switches, and interlocks, may not be used as the sole means for deenergizing circuits or equipment. Interlocks for electric equipment may not be used as a substitute for lockout and tagging procedures: a/ At the Missouri American Water Co., specifically the CP3B Switchgear Unit to Plant #3, an arc flash occurred on or about 05/14/14. The primary switch to the south bus was isolated from the unit. The high service line stemming from the north bus and running to the rear of CP3B SG was not isolated.

29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag was not placed on each disconnecting means used to deenergize circuits and equipment on which work is to be performed, except as provided in paragraphs (b)(2)(iii)(C) and (b)(2)(iii)(E) of this section. The lock was not attached so as to prevent persons from operating the disconnecting means unless they resort to undue force or the use of tools: a/ At the Missouri American Water Co., specifically the CP3B Switchgear Unit to Plant #3, an arc flash occurred on or about 05/14/14. One authorized employee was not provided with or used a padlock, suitable tag, or the equivalent for the purpose of isolating the 4,160 volts entering into the CP3B Switchgear Unit.
