29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a)  On or about May 19, 2014, the employer did not protect an employee from a fall hazards of approximately 42 feet when he climbed on the roof trusses without fall protection. 29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers:  (a)  On or about May 19, 2014, and at times prior, employees were exposed to trip and fall hazards from fourteen holes in the floor along the primary walkway in a building under construction.  The holes were 12 inches deep and 24 inches in diameter.   Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
