29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Production Areas- The employer failed to develop, document and utilize machine specific procedures to control the potentially hazardous energy on machines, such as Printing Press #2, Printing Press #3, Printing Press #4, Printing Press #5, Printing Press #6, the Pohler Cutters by Heidelberg, the Saber Cutters, and the Imagers. Without machine specific procedures the employee(s) performing maintenance and serving work were not provided with the proper information on how to secure all sources of energy such as electrical, pneumatic, hydraulic, and gravitational energy. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points: a) Bindery Department- The employer failed to ensure that all flexible cords, such as the cords supplying power to parts of the Bindery Folder #6, Bindery Folder #7, and Bindery Folder #8, were protected from pinch hazards. These flexible cords were found to be damaged and were run around equipment and on the floor in an arrangement where they could be stepped on, pinched by moving equipment or run over by materials being moved in the area.
