29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system  designed in accordance with paragraph (b) or (c) of this section:    a) On or about May 22, 2014 - at a jobsite located at 17010 Oak Park Ave., Tinley Park, IL.  - an employee installing a water main in a trench measuring approximately six (6) feet five (5) inches in depth, six (6) feet in width and twenty six (26) feet in length was not protected by cave-in protection, thereby exposing the employee to cave-in hazards.
