29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. On or about 21 May 2014, and at times prior thereto, oxygen and acetylene cylinders were being stored together without any separation, exposing anyone nearby to the hazard of being struck by flying objects from an explosion.

On or about 21 May 2014, and at times prior thereto, three iron workers were performing steel erection activities from two aerial baskets in which they all three stood on the railing of the aerial lift baskets, exposing them to a fall hazard of approximately 20 feet or more to the paved parking lot below.
