The first floor stair opening to the basement and the area at the top of the stairs on the second floor, adjacent to the stairs, were not guarded or protected. Employees were exposed to falls of approximately 9 feet. NO ABATEMENT CERTIFICATION REQUIRED

The second floor windows had a sill height of 17 inches and an interior loft opening had no railings at all. Employees were exposed to falls of 9 to 12 feet. NO ABATEMENT CERTIFICATION REQUIRED

Employees were using a job built platform supported by the forks of an all-terrain fork lift approximately 12 feet high. There were no fall protection measures in place.
