29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On May 1, 2014 at the jobsite located at or near 5401 NW 84th Terrace in Kansas City, MO, employees were exposed to an eye and face hazard in that they were using equipment with the potential for eye injury without the use of eye protection.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On May 1, 2014 at the jobsite located at or near 5401 NW 84th Terrace in Kansas City, MO, employees were exposed to a fall hazard in that they were working on a steep roof, approximately 20 feet above next lower level without an approved form of fall protection.
