This condition exposed employee to fall hazards of approximately 18 feet. 29 CFR 1926.1053(b)(1): Because of the portable ladder s length, the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in the mounting and dismounting the ladder:      (a)  Alpine Roofing Co., Inc. at 1440 Birch Street, Denver, CO 80220: On and before 10/6/14, the employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from the roof using ladders that extended 3 feet above the walking and working surface. Employee was using a portable extension-ladder for access to the roof. Employees were exposed to a fall of approximately 18 feet fall while attempting to use the ladder.      Alpine Roofing Co. Inc. was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.1053(b)(1) which was contained in OSHA Inspection Number 314658766, Citation 1, Item 2, issued on 8/19/10, and became Final Order on 9/2/10, with respect to a workplace located at 10551 Laredo Street, Commerce City, CO 80022-8771.
