29 CFR 1910.306(b)(3):  Limit switch. A limit switch or other device shall be provided to prevent the load block from passing the safe upper limit of travel of any hoisting mechanism:    a) On or about May 14, 2014 and prior to this date, employees worked with and around a Pandjiris 1144 Head Director manipulator located in the Aluminum Plant #6 Department, which had been modified to allow the equipment to weld on larger diameter tanks.  The employer had relocated the end boom arm hoist bracket and bypassed the limit switch to allow for the boom arm to rise up to higher angles which created cable failure and the boom arm to free fall.  Employees were thereby exposed to being struck by the falling boom arm and attached end equipment.
