29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.        A. Adam Elliot Construction LLC worksite located at 200 E. 2nd St., Perrysburg, Ohio: On or about May 19, 2014, the employer did not ensure employees were wearing proper eye protection while operating pneumatic nail guns when installing weatherproofing on the east side of the large commercial structure. The employee was exposed to potential flying object hazards while operating the pneumatic fastener.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.      A. Adam Elliot Construction LLC worksite located at 200 E. 2nd St., Perrysburg, Ohio: On or about May 19, 2014, the employer did not ensure an employee was protected from fall hazards while performing weather proofing activities on the north side of the 14/12 pitch roof. The employee was exposed to a fall distance of approximately 18.00 feet.     B. Adam Elliot Construction LLC worksite located at 200 E. 2nd St., Perrysburg, Ohio: On or about May 19, 2014, the employer did not ensure an employee was protected from fall hazards while performing weather proofing activities on the 14/12 pitch gable above the entry. The employee was exposed to a fall distance of approximately 11.00 feet.
