29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: The employer does not ensure that a ladder is provided in excavations for a safe means of egress. This violation was observed on or about May 22, 2014, at the job site where an employee was exposed to a fall hazard when entering and exiting an excavation without a ladder or other safe means of egress. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a safe means of egress is provided in excavations.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system: The employer does not protect each employee in an excavation from cave-ins by an adequate protective system. This violation was observed on or about May 22, 2014, at the site where employees were exposed to a crushed-by hazard when working in an excavation without a protective system to protect the employee from a cave-in. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee in an excavation is protected from a cave-in by an adequate protective system.
