29 CFR 1910.146(c)(3): When the employer decided its employees would not enter permit spaces, the employer did not take effective measures to prevent its employees from entering the permit spaces such as well head cellars: On May 21, 2014, SJL Well Servicing, LLC working at job site ZyBACH 66-5 in Hemphill County, Wheeler, Texas 75948, the employer did not ensure employees were prevented from entering confined spaces. Employees were exposed to hazardous atmosphere.
