29 CFR 1917.43(c)(5): Safety devices on powered industrial trucks were removed or made inoperative: Warehouse: On or about May 21, 2014 and at times prior thereto, the seat belt on the Mitsubishi model number FG25N, serial number AF17D02105, equipment number 546 was inoperative. (1) The seatbelt was routed in front of the seat and attached to the clasp. (2) The retraction device was not operative. This condition exposed employee to caught-between hazards in the event of a rollover incident.

29 CFR 1917.154: Compressed air for cleaning exceeded 30 psig: Warehouse: On or about May 21, 2014 and at times prior thereto, compressed air for cleaning measured 120 psig at the compressor and 50 psig at the shop-made nozzle tip. This condition exposed employees to injection and air embolism hazards in the event of contact with broken skin.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: Warehouse: North Wall, Mobile Radio Power: On or about May 21, 2014 and at times prior thereto, a flexible cord was used as a substitute for fixed wiring. The flexible cord was routed up the north wall and across and wrapped around roof joists to a 120 volt outlet on the west wall. This condition exposed employees to minor smoke-inhalation injuries in the event of fire from flexible cord failure.
