29 CFR 1926.703(a)(1): Formwork was not designed, fabricated, erected, supported, braced and maintained so that it would be capable of supporting without failure all vertical and lateral loads that could reasonably be anticipated to be applied to the form:    (a) Parking deck, Hilton Homewood Suite Hotel located at 915 Crestline Parkway, Sandy Springs, GA 30328: Employees suffered injuries while installing Hollowcore Concrete Slabs on a Diversakore steel frame as the shoring supports used were not capable of supporting a load of approximately 67,000 pounds. The employer failed to follow the shoring plan which called for 18 shoring post between the columns holding the steel beam.  The steel beam collapsed because only 8 shoring posts were used between the columns. The employer failed to install spring tension clips, lateral shoring supports and anchors into the concrete pad. The employer also failed to use shoring on a concrete pad as the plans suggest a separate plan for installation on a soil condition.

29 CFR 1926.703(a)(2): Drawings or plans, including all revisions, for the jack layout, formwork (including shoring equipment), working decks, and scaffolds were not available at the jobsite:    (a) Parking deck, new Hilton Homewood Suite Hotel located at 915 Crestline Parkway, Sandy Springs, GA 30328: The employer failed to provide all the shoring drawings, including all the revisions for the shoring jack layout. Shoring jacks were set on mud sills over gravel in the area where the steel beam failed and the collapse occurred. There was no plan available indicating that shoring could be used on a gravel surface.

29 CFR 1926.703(b)(1): Shoring equipment (including equipment used in reshoring operations) was not inspected prior to erection to determine that the equipment met the requirements specified in the formwork drawings:    (a) Parking deck, new Hilton Homewood Suite Hotel located t 915 Crestline Parkway, Sandy Springs, GA 30328: The employer failed to inspect the erected shoring system before the Hollowcore Concrete Slabs were installed. The shoring which was erected did not meet the requirements of the drawing provided. Employees installing the Hollowcore Concrete Slabs were injured during the collapse of the Diversakore Steel Beam.
