29 CFR 1926.703(a)(1): Formwork was not designed, fabricated, erected, supported, braced and maintained so that it would be capable of supporting without failure all vertical and lateral loads that could reasonably be anticipated to be applied to the form:    (a) Parking deck, Hilton Homewood Suite Hotel located at 915 Crestline Parkway, Sandy Springs, GA 30328: Employees suffered injuries while installing Hollowcore Concrete Slabs on a Diversakore steel frame as the shoring supports used were not capable of supporting a load of approximately 67,000 pounds. The employer failed to follow the shoring plan which called for 18 shoring post between the columns holding the steel beam.  The steel beam collapsed because only 8 shoring posts were used between the columns. The employer failed to install spring tension clips, lateral shoring supports and anchors into the concrete pad. The employer also failed to use shoring on a concrete pad as the plans suggest a separate plan for installation on a soil condition.
